This form is REQUIRED for all concert and jazz band students. This form must be filled out and returned to Mr. Bennett by Friday, August 12. (This form can be filled out electronically and printed which is highly encouraged.) |

This form is REQUIRED for all concert and jazz band students. This form must be filled out and returned to Mr. Bennett ASAP. Marching band students who have already turned one in - DO NOT - need to do another. The proof of insurance page must have your signature notorized or witnessed by Mr. Bennett, Mr. Nelson, or Mr. Bitter. We are available before and after school. If you wish or need to purchase school insurance, please contact Mr. Bennett. (This form can be filled out electronically and printed which is highly encouraged.) |

Use this form to support your student through a tax credit donation. Be sure to follow the instructions on the form. Checks must be payable to Gilbert Public Schools and the NAME on the check must match the NAME on the Form. Cash, Cashier's Check, or a Money Order are also acceptable forms of payment to turn in with the tax credit form. |
